Action Steps for Certification

PRECERTIFICATION
Community initiatives seeking to build a Certified Pathways Community HUB are required to complete a precertification application. This process helps to assure there are no existing HUBs within the designated service region, appropriate community engagement and related prerequisites of HUB Certification are completed. Upon notification of the Pathways Community HUB Certification Program (PCHCP) of the intent to complete precertification the PCHCP forwards a precertification application packet to the proposed HUB site. The packet includes information about the certification process, which is intended to help acquaint the applicant with the requirements of certification (i.e., precertification pre-requisites, standards, site visit, review and assessment of documentation to determine compliance with standards).

CERTIFICATION

PRE-SITE VISIT ACTION STEPS
Step 1. Following completion of the precertification an application for Certification is submitted.
Step 2. Site reviews application materials and requests Technical Assistance.
Step 3. A technical assistance team is assigned and provides consultation to the applicant.
Step 4. Site prepares application for submission to the HUB Institute.
Step 5. The HUB Institute acknowledges receipt of the application via correspondence to the applicant.
Step 6. The HUB Institute notifies the certification team.
Step 7. The Certification Team reviews the application, contacts the applicant to schedule a conference call to answer questions, and confirms a site visit date to assess documentation of HUB compliance with the standards.

SITE VISIT AND ASSESSMENT ACTION STEPS
Step 8. The Certification Team conducts the site visit.
Step 9. The Certification Team completes site visit and documents findings on site visit form.
Step 10. The Certification Team discusses any issues warranting clarification with the sites and creates a Quality Improvement Plan.

DETERMINATION OF CERTIFICATION STATUS ACTION STEPS
Step 11.  The Certification Team decides whether the site meets qualifications for certification. If a determination is made that the HUB meets the certification criteria, then the level of certification is determined. If provisional certification is determined, then the Certification Team prepares the feedback form and a Quality Improvement Plan.
Step 12. The Evaluation Review Panel (ERP), comprised of external industry leaders, is convened to review the Certification Team’s recommendations for HUB certification. The ERP reviews the recommendations of the Certification Team and then determines the HUB’s certification status and designation level.
Step 13. The Certification Team discusses the feedback and score to ensure understanding of the certification status.

CERTIFICATION STATUS DESIGNATION ACTION STEPS
Step 14. The HUB Institute presents the HUB with the HUB certification designation and certificate.
Step 15. If a determination is made that the HUB does not meet the certification criteria, then a feedback form is completed with Quality Improvement Plan and score with application.
Step 16. Technical Assistance Team provides consultation and Technical Assistance to the site.
Step 17. The HUB applicant will be eligible to repeat steps 4-11 in twelve months.
