ACTION STEPS FOR PROVISIONAL CERTIFICATION DESIGNATION

HUBs that have received provisional certification designation must work through their HUB Action Plan Agreement within the timeline documented which should be not greater than 12 months. If the action steps have not been completed within the documented timeline HUBs may submit an Appeal to request an additional 6 months. No additional fees are assessed to complete this process within the 12-month period or during an extended six-month period granted by the Evaluation Review Panel. If this process has not been completed and the HUB does not achieve full certification designation, then the applying HUB can restart the process within two months. They will need to re-start the entire certification process and will be assessed the full fee for certification review.

ACTION STEPS FOR HUBS NOT MEETING PROVISIONAL CERTIFICATION

HUBs that have received provisional certification designation have up to 12 months to repeat steps 4-17 to complete the certification process. If the action steps have not been completed within the 12-month period, HUBs may submit an Appeal to request an additional 6 months. No additional fees are assessed to complete this process within the 12-month period or during an extended six-month period granted by the Evaluation Review Panel. If this process has not been completed to be considered for full certification designation, then the applying HUB will need to re-start the entire certification process and will be assessed the full fee for certification review.
